Flue shot clinic

Sibley County Public Health and Human Services is working with GFW Public Schools to administer the annual flu vaccination at school. This vaccine will protect against four influenza strains that are expected to circulate this year.

Clinic Date:  Wednesday, October 11, 2023 

  • Morning: GFW Elementary School
  • Afternoon: GFW Middle School and High School

Return Consent form(s) and copy of insurance card or payment to the school by: Monday, October 9.

Complete the 2023-24 Influenza Vaccine Consent form (Spanish version here) (front and back).  

  • Provide a copy of your health insurance card (front and back).  This allows us to bill your health insurance company on your behalf.  Please call the customer service number on the back of your health insurance card to make sure the influenza vaccine is covered & any necessary deductible has been met.

  • If the person receiving the vaccine does not have insurance coverage, we do request a $20.00 administration fee.  (the $20.00 fee is payable by cash or check made out to Sibley County PHHS).

We request that clothing that allows easy access to the upper arm be worn.

Your health care provider also can answer your questions about the influenza virus and will be able to give you or your child the seasonal influenza vaccine.
